DECLARATION AND AGREEMENT. I/We, the undersigned, hereby declare that to the best of our knowledge and belief the information furnished in this application is true and correct, and that if found to be false or misleading in any respect, this application may be invalidated and the applicant's registration terminated.

I/We accept liability for damage to Institute property caused by the Applicant, and indemnify the Institute against any loss, damage, or claim arising in respect of property left at the Institute, or arising from the Applicant's participation in sporting or other activities, acknowledging this is at the Applicant's sole risk.

I/We acknowledge that a Minimum Initial Payment (MIP) is required by a set date each year unless satisfactory arrangements have been made with the Institute, and agree to pay interest on overdue fees at 2% per month compounded monthly.

I/We shall abide by all regulations of the Institute, and the Applicant shall be under the Institute's disciplinary control from the date studies, orientation, or registration begins, until the Institute accepts a notice of withdrawal or the Applicant fails to renew registration.

I/We accept that the Institute keeps this Declaration and related documents electronically, and that electronically generated documents replace originals signed by me. The Institute may report to a parent, guardian, or fee contributor on matters of conduct, academic progress, well-being, or health as it deems necessary.
